1) IT Service Desk > IT Incident / Service Request

 

2) Select Category before submit the report.

 

3) Read the display article before submit a report

 

4) Click on "Continue to Manage IT Service Desk" if the article could not help with the question

 

5) Fill in the all the details at the report form

 

6) Click on "Choose File" to upload a document before submit the report

 

7) Click "Submit" after fill in all the required details

 

8) Report with ID displayed

 

9) User received email
